OFFICE HOURS:

Normal business hours are Monday through Friday, 8:30am to 5:00 pm. The office is closed from 12:00 noon
to 1:00 pm for lunch. During May through August, our Friday hours are limited to mornings only. The office is
closed on weekends and Christian or government holidays.

Patients are seen by appointment only. For follow-up visits and non-emergencies, it is best to schedule your
appointment several weeks in advance. To cancel appointments, please notify our office at least 24 hours prior
to your scheduled appointment time in order to allow the staff to reschedule another patient who may been in need.
You may notify our office via phone, email or fax.

Directions: I-94 to Exit 27. Head south on Niles Road and turn right onto Hollywood Road. Head south on
Hollywood Road to the first intersection. Turn left onto Maiden Lane. Turn right onto Stonegate Park.

PAYMENT OPTIONS:

Payment is due at time of service. We accept most major credit cards, cash and personal checks. Charges for
insufficient funds received are detailed in the new patient packet. If a patient fails to make payment on the account
at the time of visit, s/he may not be seen for follow-up until the balance has been addressed.

We currently participate with Medicare and most local health plans. You MUST bring your insurance cards and/or
authorizations with you and present them at each visit.

If your visit is covered by health insurance, we may file your insurance claim. If we participate in your insurance plan,
the cost of the office visit may be limited to the co-pay. We are required by law to collect all deductibles and
co-insurance from our patients.

Please note that there is a contract between you and the your health insurance provider. Genesis has no legal
relationship with your insurance carrier. The actual burden of payment is on the patient. You have contracted an
insurance carrier to cover services detailed in your contract with them.
