Thank you for choosing the Genesis Family Health Center as your new primary care provider
Enclosed you will find the necessary paperwork to establish yourself as a “NEW PATIENT” in our office. Please be sure to bring these completed forms, along with your insurance cards, to the office at the time of your appointment.
It is your responsibility, as a new patient to our office, to contact your insurance company and let them know you have chosen us as your primary care provider. Because there are so many different types of insurance plans that we deal with, failure to contact your insurance company may result in a financial burden placed upon you. If you have questions regarding this, please feel free to ask one of our office staff to assist you.
Included in this packet of information is an authorization for use and disclosure of protected health information, consent notice, health history and your rights and responsibilities. Please complete all forms prior to your appointment. If you are requesting a transfer of health records, please notify your current physician immediately. This will allow your provider to copy your records and mail them to our office before you are seen at your first appointment.
Please note that if you are unable to come to your scheduled “NEW PATIENT” appointment, you must call to cancel and reschedule. If you do not notify the office that you are canceling the appointment, you will not be rescheduled in the office. It will be your responsibility to choose another physician and notify your insurance company of the changes.
